1. Voluntary Participation & Details
I, the undersigned, acknowledge that I have voluntarily chosen to have my child participate in the Cartoon Coach AFTER SCHOOL SPECIAL workshop conducted by Cartoon Coach with instructor: Jason Flores-Holz. The Cartoon Coach AFTER SCHOOL SPECIAL workshop is an ongoing weekly workshop that takes place at the Real Estate Shop, 1033 College St. W., Toronto, Ontario, Canada.
2. Assumption of Risk
I understand that while drawing is generally a safe activity, participation inherently involves the use of specialized tools and materials, which may include, but are not limited to: paper, sharpened pencils and other art supplies. I acknowledge that these materials carry risks including, but not limited to minor cuts or scrapes, skin irritation, clothing damage, and property damage. I voluntarily and freely assume all risks associated with my child's, participation in the Cartoon Coach AFTER SCHOOL SPECIAL workshop.
3. Release of Liability
I hereby release, waive, and forever discharge Cartoon Coach, their instructors, employees, agents, and affiliates from any and all liability, claims, or demands for personal injury, property damage, or any other loss arising out of my child's participation in this Workshop, except in cases of gross negligence or willful misconduct by Cartoon Coach.
4. Code of Conduct & Medical Authorization
I agree to follow all instructions and safety guidelines provided by the Cartoon Coach AFTER SCHOOL SPECIAL workshop instructors. In the event of an emergency, I authorize Cartoon Coach (Jason Flores-Holz) to secure necessary medical care, and I agree to be financially responsible for any and all costs associated with such medical attention.
5. Photography & Media Release
I consent to Cartoon Coach taking photographs or videos of my child and their artwork during the Cartoon Coach AFTER SCHOOL SPECIAL workshop. I grant Cartoon Coach the right to use these materials for marketing, website, and social media purposes.
